    Sleeve Gastrectomy Surgery

    Duodenal Switch / Single Anastomosis Duodeno-ileostomy with Sleeve (SADI-S)

    The Duodenal Switch (DS) is a weight loss operation that combines the restrictive quality of the sleeve gastrectomy and the malabsorptive quality of the gastric bypass. This operation is the most drastic operation we perform with the greatest amount of weight loss and highest resolution of diabetes mellitus.
    The DS is a two step operation which consists of
    1. Sleeve gastrectomy (insert sleeve link) and
    2. Duodeno-ileostomy where the small bowel is disconnected from the stomach and reconnected further down. By performing the second part of the operation, we direct food that would normally go through the entire intestine and bypass it to the end of the intestine. This single anastomosis technique creates fewer complications and safer weight loss than its predecessors.

    How does the DS operation work?
    The DS works via several mechanisms throughout the gastrointestinal tract. It provides restriction to food intake, reduces absorption and alters the hormones of the GI tract. The restriction comes as a result of the sleeve gastrectomy. This part of the operation also reduces ghrelin (hunger hormone), increases insulin sensitivity and fat utilization.

     

    Am I a candidate for loop DS?
    Generally speaking, the loop DS is reserved for patients with a BMI >50 and diabetes mellitus that has been poorly controlled. Data has shown that
    there can be up to a 95% resolution of diabetes with this operation. Potential patients should keep in mind that the success of any weight loss/metabolic surgery is contingent upon good follow up and durable lifestyle changes.

    I’ve had bariatric surgery already. Can I still have a loop DS?
    For patients that have had prior bariatric surgery, an in-depth evaluation is needed to determine if a revision of the prior surgery is needed to optimize weight loss and overall health. This type of evaluation would require meeting with a comprehensive team that includes your surgeon, dietitian, LPC or psychiatrist, and other physicians involved in your care.
